Professional referrals
For professionals to refer a young person to our 1-1 intervention services.

Before you submit this form you must:
1. Read our Referral Form Privacy Notice. This explains what we collect, why, who sees it, how long we keep it, and the rights of the young person and their family.
2. Share the privacy notice with the young person and their parent or carer before submitting. You can share the Referral Form Privacy Notice or the Young Person’s Privacy Notice (which is written in plain language for young people).
3. Give them a chance to ask questions before you click submit.
If the young person or their family objects to the referral, please contact One-Eighty to discuss before submitting, unless there is an immediate safeguarding concern.
